Grampians Health – Stawell Community Services are currently seeking to appoint a highly motivated experienced Registered Nurse to join our Care Coordination team

 

Care Coordinator
Fixed Term Full Time (7 month contract)

Post-Acute Care (PAC) and Hospital Admission Risk Program (HARP)

 

About the Role:

The vacancy is for Post-Acute Care three days per week and Hospital Admission Risk Program two days per week.

The Care Coordinator works with consumers to promote health independence and positive approaches to recovery from acute illness and managing chronic health conditions.

The role provides integrated and coordinated person centered care to improve client health outcomes and reduce hospital length of stay and avoidable admissions.

 

About You:

The preferred applicant would have a strong interest in working with people in the community and with clients that have complex health care needs and chronic conditions.

The role would suit a self-motivated, team focused individual with a commitment to providing high quality holistic care to clients in the HARP and PAC programs.

Knowledge of chronic disease management and demonstrated experience of effective communication skills with healthcare providers will be valued.
