Job Description
This is your opportunity to join the dynamic team in Community Nursing and Home Support at East Grampians Health Service, and provide assistance to clients to maintain their independence and provide ongoing domestic, personal and social support to people living in the community. In addition, you will support carers of people who are frail aged including people with dementia and people with a disability or mental health condition.

Community Nursing is a part of the broader Community Services Department based in Ararat. We provide community nursing across the catchment area of Ararat Rural City. Community nursing includes palliative care, district nursing, hospital in the home, hospital admission risk program (HARP), home care packages and specialist nursing programs. Each program area aims to provide awareness and education which enables community members to have a better understanding of their own health issues and thus take greater responsibility for improving their own health outcomes.

About the Role
The role will incorporate travel to clients’ homes and various locations within the region involving but not limited to:
- Personal care, showering, dressing and cooking
- Assist with medications
- Domestic services; cleaning, grocery shopping and socialising
- Escort to medical appointments
Our ideal candidates will
- be caring and compassionate in nature
- have a high level of attention to detail
- be committed to confidentiality and display a high level of integrity
- hold a Certificate III in Aged Care/Home and Community Care,
- have a current First Aid Certificate and
- a current and valid Driver’s licence
Appointment is subject to a satisfactory immunisation clearance, current and satisfactory Employment Checks (Police and Working With Children Checks).
