Central Grampians Palliative Care is located at East Grampians Health Service (EGHS) Ararat and is the primary provider of specialist palliative care services in the local government areas of Ararat, Pyrenees and Northern Grampians.
Our team provides patient centred, evidence based specialist services for consumers with palliative and end of life care needs, supporting consumers and their carers to feel safe and comfortable. Our team works closely with the Grampians Regional Palliative Team and is working closely with Ballarat Hospice to improve access to after-hours care and advice. EGHS also participates in the Palliative Care Outcomes Collaboration (PCOC) to benchmark and continually identify opportunities to improve our service.
If you have:
- Recent experience and qualifications in palliative care nursing;
- Demonstrated management experience;
- A commitment to the provision of care which focuses on the individual and support networks, and;
- The ability to contribute and practice collaboratively in a multidisciplinary team;
