Clinical Nurse Consultant A – B (ZF4 – ZJ4)
Permanent Full Time
(80 hrs per fortnight) with ADO

An exciting opportunity exists for an innovative, positive Registered Nurse to make a difference in the lives of our Aged Care residents as the RIR Coordinator. This permanent full time position is available for an immediate start, with the opportunity for negotiable hours. Job share will be considered, please indicate in your application how many hours you’re applying for. The Residential in Reach Coordinator drives excellence in clinical standards and quality of care in an advanced clinical role in the area of the Older Person (chronic and acute conditions). The successful applicant will work as part of a multidisciplinary team to facilitate and provide high quality care to residents of residential aged care facilities throughout the Wimmera.

Key Selection Criteria include:

  • Registered Nurse with current AHPRA registration
  • Current Victorian Driver’s license
  • Demonstrated assessment and management skills at an advanced practice level in the nursing care of an Older person including acute deterioration of chronic health conditions
  • Demonstrated effective leadership skills
  • Highly developed communication/interpersonal, negotiation and conflict solution skills
  • Demonstrated commitment and ability to work in a multi-disciplinary team
  • Demonstrated ability to be self-driven, and work autonomously
  • Experience in risk management and quality improvement processes and programs, or willingness to learn
  • Well-developed computer literacy skills using various applications
  • Demonstrated commitment to professional development and learning for self and other team members
  • Relevant post-graduate qualification, or working towards / willingness to do so
